Isaac demonstrates how quick and easy it is getting sharp extrusions. The workflow he covers uses masking, poly groups, group loops, transpose, and finally ZModeler for the smaller details.

Certainly, there are a few ways to create a sharp extrusion in ZBrush. When using masks however, there is a small setting that will allow you to get sharp extrusions by altering the numbers slightly.
